In the last 30 years of lumbar fusion – open posterior lumbar fusion technologies – we have experienced several gener- ations of posterior fusion implants from the first threaded cages and thereafter to the first impacted cages to surface modified PEEK cages or 3D printed titanium cages. Several biomaterials from Titanium to PLAs to PEEK to PEKK and back to Titanium with or without autologous, allogenic or synthetic cells or growth factors have piqued great interest to improve spinal fusion. Posterior support approach and preparation techniques have improved in a similar way, becoming systematically more anatomy focused, and less destructive (less tissue and nerve damage) and minimal invasive approaches stimulating repair and reconstruction have emerged. The technological and innovative advancements of the supporting instruments have enhanced knowledge, experience and generated evidence in a more natural way like a normal learning curve.
